Please do not submit patient or health information through this website; our form is for clinic owners and staff. When we provide services to healthcare clients, any protected health information is handled under a business associate agreement with that client and only through tools covered by such agreements. We do not place advertising tracking pixels on patient-facing pages we build for clients.

We keep information only as long as needed for the purposes above and use reasonable safeguards to protect it. No method of transmission or storage is completely secure.
